Acentra Health is looking for a PASRR Evaluator to join our growing team.

 

Job Summary

Join Acentra Health as a Clinical Evaluator (PASRR) in Los Angeles, California. In this role, you will conduct PASRR Level II Pre-Admission Screening and Resident Review assessments across nursing homes, hospitals, and community-based settings throughout Los Angeles. You will help determine appropriate placement and level-of-care recommendations for individuals with complex needs, supporting improved health outcomes.

This PRN hybrid role offers flexibility and purpose, combining remote work from your home office with local travel as needed. All travel expenses including mileage and parking are covered and reimbursed by Acentra Health. In addition, Acentra provides all necessary technology and equipment for your home office, including a laptop for use in the field.

Responsibilities:

• Complete accurate PASRR screenings evaluations, determining the appropriate level of care based on patients’ physical, behavioral, and psychological functioning. 
• Review medical records and collaborate with healthcare professionals, patients, families, internal partners and support systems as necessary, to develop clinically sound, well-informed level-of-care recommendations based on the evaluations, while tracking personal performance metrics and maintaining compliance with established turnaround time requirements 
• Ensure compliance with necessary workflow, including re-review of records based upon input from the psychiatris
• Knowledge of standards of practice related to the determination of Level of Care and standards of practice related to mental health, geriatrics, and other disorders commonly necessitating long-term care placement.
• Maintain compliance with established workflows, regulatory requirements, and best-practice standard
• Leverage technology, computer equipment, web-based software, and our proprietary platforms to efficiently manage and track assessment activities.
• Understand and adhere to all corporate policies, including those related to HIPAA and its Privacy and Security Rules.

Qualifications:

Required Qualifications, Knowledge, and Experience  

• Must possess an active, unrestricted California license in a healthcare or related field, such as Licensed Professional Clinical Counselor (LPCC), Licensed Mental Health Counselor (LMHC), Licensed Marriage and Family Therapist (LMFT), Licensed Clinical Social Worker (LCSW), Nurse Practitioner (NP), or Psychiatric Mental Health Nurse Practitioner (PMHNP).
• Graduation from an accredited college with a relevant degree program and a minimum of 3 years (within the last 5 years) of direct experience providing mental health services. This includes managing complex differential diagnoses related to substance use, medical conditions, cognitive issues, and dementia.
• Minimum of 1 year of experience working with individuals with intellectual disability or related conditions.
• Knowledge of the organization of medical records, medical terminology, and disease processes required.

• Excellent communication skills, with the ability to clearly convey complex information to individuals and groups through verbal, written, and electronic formats.

• Strong clinical assessment and critical thinking skills

• Experience with medical record abstraction

Preferred Qualifications

• Knowledge of psychiatric disorders as defined by the most current Diagnostic and Statistical Manual in use in applicable state.
• Knowledge of Federal Regulation 42 CFR, Chapter 483.
• Knowledge of standards of practice related to mental health, intellectual and developmental disabilities, geriatrics, and other disorders typically requiring long-term care services and support.

• Experience with public mental health and medical programs, including familiarity with public sector services and community resources, is preferred.

• Experience working with geriatric populations is preferred, particularly within long-term care, behavioral health, or community-based service settings.

• Skilled in using Microsoft Office Suite; Excel, Word, PowerPoint, Outlook, and Teams, as well as other web-based tools and platforms
• Attention to detail and the ability to effectively prioritize and execute tasks in a timely manner.

Compensation

The compensation for this role is $41.00 - $42.00 per hour base

Based on our compensation program, an applicant’s position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
